Children participating in general assessment 1A (“Baseline”) received four questionnaires: the 1A General Questionnaire, the 1A Birth Questionnaire, the 1A Behavior Questionnaire and the 1A Food Questionnaire. These questionnaires were filled in by the parents.

Children did not receive any follow-up questionnaires between assessment 1A and 2A.

The 1A Food Questionnaire contained the following sections and subsections:

• Personal data

• Macronutrients (FFQ-h)
• Diet (general)
